Volunteer/Parental Requirements (For volunteers under the age of 18)
It is the responsibility of the volunteer and his/her parent to read over, sign and return the volunteer checklist of duties and important expectations to the volunteer coordinator, to ensure full understanding of, and compliance with all volunteer requirements.

It is also the responsibility of parents/guardians of volunteers 12-17 years of age to provide and ensure that the volunteer is transported to and from the volunteer site.

I hereby agree to abide by all requirements stated and understand that Sisters United In Human Service, Inc. will be waived of all responsibility in, or affected by these requirements.
